Day habilitation is a pathway to greater independence, social connection, and personal growth for individuals with intellectual or developmental disabilities.
Sovereign Care Services provides structured, community-based day habilitation across Indianapolis, focused on teaching essential life skills, fostering community integration, and developing self-advocacy. Funded through Indiana Medicaid waiver programs, it offers meaningful daily engagement as a vital alternative to isolation or institutionalization.
Your goals drive the program. We design activities around personal interests, whether that is learning to cook, exploring local history, or volunteering with animals, and we teach skills in the actual environments where they will be used so they hold up in daily life.
Hands-on training and practice in the practical skills independence requires: cooking, hygiene, money handling, transportation, and managing a schedule.
Support to become an active, connected member of the community through outings, volunteering, recreation, and local partnerships.
Building interpersonal skills, self-expression, problem-solving, and the confidence to self-advocate in everyday situations.
Specialized support integrated into the day, including positive behavioral strategies, so needs are addressed holistically.
Individuals decide how their day is spent, from daily activities to long-term goals, with staff coaching rather than directing.
Genuine friendships and community networks that reduce reliance on formal services and make daily life richer.
Many families combine weekday day habilitation with in-home support, so the skills built in the community carry over at home, evenings and weekends included.

My MIL, who was still working full-time, had an acute stroke affecting her memory. A current client referred us to Nataya at Sovereign Care. She and the staff are very knowledgeable. I appreciate their compassion and patience with my family's learning curve. This staff has definitely been a blessing during a time in need.
They truly care about the client. They help find resources for any individual problem and will fight for you to make sure quality of life is met. We had an incident where Medicaid wasn't giving the client all of her hours. Sovereign helped us fill out the forms and submit them. I got my hours paid for.
My sister, who has Down syndrome, requires patient and personalized support. Sovereign didn't just assign a caregiver, they matched her with someone who celebrates her strengths and encourages her independence. It is more than a service. It is a relationship built on respect.
Wonderful care professionals. I never had an issue. Communication is great. They genuinely care about the families they help. I'll definitely recommend them to anyone.
Adults with intellectual or developmental disabilities who are eligible for or enrolled in Indiana BDDS waiver programs, including the Community Integration and Habilitation (CIH) Waiver. It suits individuals not currently in competitive employment who want to build skills for greater independence.
Adult day services provide supervised, social daytime care. Day habilitation is goal-driven: every activity ties back to a skill in the service plan, and much of it happens out in the community rather than in one building.
A blend of targeted skill-building sessions and community time: cooking or budgeting practice in the morning, a library, volunteer shift, or fitness outing in the afternoon. Schedules are built per person, not per group.
Most families use Indiana Medicaid waiver funding through BDDS. We help you and your service coordinator confirm eligibility and complete the paperwork. Private pay is also available.
Across Indianapolis, we use the community as the classroom: libraries, parks, stores, gyms, volunteer sites, and local businesses, plus dedicated space for focused skill work when that fits the goal better.
