Sovereign Care Services® is pleased to announce that it has earned CARF accreditation
Structured Family Caregiving program (SFC) offers a financial stipend and ongoing coaching to caregivers, clients families that deliver care at home for seniors and disabled persons. You may meet SFC requirements and be eligible for financial stipend if:
1. You live with the person as the primary caregiver; who may be a non-family member (foster care) or a family member
2. The person you take care of is Indiana Medicaid-eligible and participates under the BDDS or Aged & Disabled waiver program.
The first step in making a great decision about community living care is getting the information you and your loved ones need.
Once onboarded, caregivers are teamed with one of our registered nurses to assess the client's needs and provide a service plan for client care, medication reminders and health care coordination with other providers.
Caregivers are also partnered with a coaching specialist for training as you gain the knowledge necessary to provide daily supports to your loved one. Coaching, training and ongoing caregiver guidance help our clients to age in place, in a setting that is both familiar and comfortable to them. We assist our caregivers to thrive and provide the highest quality of care for our clients in addition to balancing their own well-being.
